Please make sure that you print out both pages. Complete the entire form. Indicate at the top which camp you are interested in sending your child. Sign and date the application before submitting it to the school office.
Please note that the registration fee of $50 and two weeks of summer camp should accompany all applications.
The entire form must be completed. The Medical form must be signed and dated by a parent or legal guardian and doctor. The Medical form must be submitted prior to June 20th. For camper's safety, no one is allowed to attend camp unless this form is received before this above date.
